Preventive Medicine Physician

About This Role

This role involves conducting in-home health evaluations for Medicare Advantage, Medicaid, and certain commercial plan members. It suits physicians interested in flexible hours, connecting with people one-on-one, and helping them re-engage with their health, without prescribing medicine, ordering lab tests, or altering current treatment regimens.

Responsibilities

• Conduct in-home visits that focus on a person's overall health and well-being and identify potential barriers to care
• Perform medication and medical history reviews
• Conduct a physical evaluation in the health plan member's home
• Make referrals to the individual's health plan
• Encourage the participation of loved ones
• Ensure PCPs are aware of their patients' health and overall quality of life

Requirements

• Physician (MD/DO) contractor
• State License Required
• Medicaid License or willingness to enroll (with our support) strongly preferred
