About the role
We are seeking a licensed Physical Therapist to provide personalized, one-on-one care to older adults in senior living communities, private residences, and virtual settings. Key Responsibilities Include
- Performing evaluations, developing individualized treatment plans, and delivering evidence-based therapy to support mobility, independence, and wellness.
- Documenting care accurately in electronic medical records in compliance with regulations.
- Collaborating with interdisciplinary teams including PTs, OTs, SLPs, and fitness specialists.
- Educating patients, caregivers, and staff to promote safety and aging-in-place strategies.
- Utilizing telehealth and virtual care to enhance service delivery and flexibility.
Qualifications And Skills Required
- Valid Physical Therapy license in applicable state(s) or eligibility to obtain one.
- Degree from an accredited Physical Therapy program.
- Proficiency in basic computer skills and electronic medical records.
- Current CPR certification.
Physical demands include: exerting moderate effort, assisting with patient transfers, repositioning patients, walking, climbing stairs, and driving between locations.
