Role: Per Diem Orthopedic Home Physical Therapist
Responsibilities: Evaluate and treat patients in their homes recovering from orthopedic surgeries, including total hip and knee replacements. Provide patient education on mobility, transfers, and safety, and develop individualized treatment plans. Conduct assessments, document care, and collaborate with interdisciplinary teams. Facilitate functional training, including ADLs, IADLs, and community reintegration. Ensure safe, effective interventions in compliance with protocols, and manage billing and documentation using EMR software. Communicate with team members to coordinate care and schedule evaluations.
Qualifications & Skills: Bachelor's or Master's degree required; Doctorate preferred. Licensed in the practicing state with valid CPR. Strong orthopedic experience, especially in post-surgical rehab. Excellent communication, clinical decision-making, and computer skills. Must demonstrate professionalism, teamwork, and dedication to patient success.
