Continuum Therapy Partners is hiring a Licensed Physical Therapist Assistant (PTA) for a full-time opportunity at Norwood Crossing in Chicago, Illinois. As a Physical Therapist Assistant (PTA), you"ll work under the direction of a licensed Physical Therapist while collaborating with Occupational Therapists, Certified Occupational Therapy Assistants, Speech-Language Pathologists, nursing staff, and other rehabilitation professionals to deliver exceptional patient-centered care.
Physical Therapist Assistant (PTA) Responsibilities
- Provide skilled physical therapy treatments under the supervision of a licensed Physical Therapist.
- Assist patients with gait training, transfer training, therapeutic exercise, balance activities, strengthening, and functional mobility.
- Monitor patient progress and communicate changes to the supervising Physical Therapist.
- Complete accurate and timely electronic documentation.
- Collaborate with physicians, nursing staff, caregivers, and the interdisciplinary rehabilitation team.
- Educate patients and families on mobility, safety, and home exercise programs.
- Maintain compliance with Medicare, state, federal, and company guidelines.
Qualifications
- Graduate of an accredited Physical Therapist Assistant program.
- Current Illinois Physical Therapist Assistant (PTA) license required.
- Skilled nursing, long-term care, rehabilitation, post-acute care, or outpatient experience is helpful but not required.
- New graduate PTAs are encouraged to apply.
- Strong communication, teamwork, and organizational skills.
