Physical Therapist - Acute Therapy

We are seeking a licensed Physical Therapist to evaluate and treat patients of all ages, developing personalized care plans that incorporate goals, treatment, outcomes, and discharge strategies. The role involves performing patient assessments, selecting appropriate examination methods, and modifying interventions based on patient response and evidence-based practices. Key Responsibilities Include

• Designing and adjusting plans of care to ensure effective rehabilitation outcomes.
• Educating patients, families, and healthcare teams using effective teaching methods.
• Performing safe patient transfers and demonstrating proper body mechanics.
• Utilizing standardized outcome measures and documenting care in compliance with organizational standards.
• Supervising support personnel and providing feedback on care delivery.
• Communicating patient progress, changes in status, and discharge recommendations clearly to the medical team.
• Participating in ongoing education, especially related to acute stroke care, and complying with licensing and certification requirements.

Qualifications Include

• Bachelor's Degree in Physical Therapy.
• Licensed Physical Therapist (PT) in Illinois.
• Basic Life Support (BLS) certification within 30 days.
• Ability to perform patient transfers and demonstrate safe, respectful patient care.
• Excellent communication, clinical evaluation, and documentation skills.
