Full-Time Physical Therapist (PT) – Skilled Nursing & Rehabilitation

Deer Meadows Rehabilitation Center is seeking a compassionate and motivated Full-Time Physical Therapist (PT) to join our in-house rehabilitation team in Philadelphia, Pennsylvania . Our therapists work closely with nursing, physicians, and interdisciplinary staff to provide outstanding, patient-centered care in a supportive Skilled Nursing and Rehabilitation environment.
Position Responsibilities
- Evaluate residents and develop individualized physical therapy treatment plans.
- Provide skilled physical therapy services to short-term rehabilitation and long-term care residents.
- Establish functional goals that promote mobility, strength, balance, endurance, and independence.
- Collaborate with physicians, nursing staff, and the interdisciplinary team to ensure optimal patient outcomes.
- Educate residents and families regarding treatment plans, mobility techniques, and discharge recommendations.
- Complete accurate and timely documentation using Net Health and PointClickCare (PCC).
- Maintain compliance with all federal, state, and company regulations.
Qualifications
- Current Physical Therapist (PT) license in the Commonwealth of Pennsylvania.
- Graduate of an accredited Physical Therapy program.
- Skilled Nursing Facility (SNF) or Long-Term Care experience preferred, but new graduates are encouraged to apply.
- Experience with Net Health and PointClickCare (PCC) is a plus.
- Strong communication, clinical reasoning, and organizational skills.
- Ability to work collaboratively in a fast-paced rehabilitation environment.
