Pediatric Registered Nurse

About This Role

This In-Home Pediatric Registered Nurse role provides high-quality, individualized nursing care to pediatric patients in the comfort and safety of their homes. The In-Home Nurse works closely with patients, families, physicians, and other healthcare professionals to manage ongoing medical needs, promote health and development, and support families in delivering safe and effective care. The ideal candidate is committed to providing compassionate care and creating a safe, supportive environment for children with complex medical conditions, chronic illnesses, developmental disabilities, or special healthcare needs.

Responsibilities

• Provide skilled nursing care to pediatric patients in their homes according to individualized care plans and physician orders
• Conduct comprehensive nursing assessments, monitor patient conditions, and identify changes in health status that require intervention or medical follow-up
• Administer prescribed medications, including oral, enteral, injectable, and IV medications, as applicable to the patient's care plan
• Monitor and document vital signs, respiratory status, nutritional intake, pain levels, and other relevant clinical indicators
• Provide care and management of feeding tubes, including G-tubes and J-tubes, as applicable
• Provide tracheostomy care, suctioning, oxygen therapy, and respiratory treatments for patients with complex medical needs, as applicable
• Assist with ventilator-dependent or other medically complex pediatric patients according to training, physician orders, and agency protocols
• Perform wound care, catheter care, ostomy care, and other skilled nursing procedures as required
• Provide enteral feeding and nutritional support while monitoring for complications and changes in patient condition
• Educate parents and caregivers on medication administration, medical equipment, infection prevention, nutrition, disease management, and emergency procedures
• Support families in developing confidence and skills to safely participate in their child's daily care
• Maintain accurate and timely documentation of all nursing services, assessments, interventions, medications, and patient responses in the EMR/EHR system
• Communicate regularly with physicians, case managers, therapists, and other members of the healthcare team regarding the patient's progress and changing needs
• Recognize and respond appropriately to changes in the patient's condition, including initiating emergency procedures and contacting appropriate medical personnel when necessary
• Maintain a clean, safe, and infection-controlled home care environment
• Follow HIPAA, infection-control, medication-safety, and other applicable healthcare regulations and agency policies
• Advocate for the patient's comfort, safety, dignity, and overall quality of life
