Patient Care Manager

About This Role

This Patient Care Manager role is a key part of the management team within the Stroke and Post Acute program, reporting to the Program Director of the RCC, Continuing Care, Rehab and Stroke Programs. The manager is responsible for coordinating and directing the A2 Orange Complex Care and A1 Aqua Palliative units, including their human and fiscal resources. The role involves collaborating with various healthcare professionals to ensure high-quality, effective, and efficient services and care, and is accountable for planning, implementing, and integrating plans aligned with the program and organizational vision.

Responsibilities

• Manage the operation and organizational effectiveness of the department, including human and financial resources
• Collaborate with the team in developing, implementing, and managing short and long-term strategic priorities and quality improvement initiatives
• Contribute to the organization and program vision, values, and strategic direction
• Support and maintain the principles of shared governance, interdisciplinary and collaborative management, patient-focused service, and continuous quality improvement
• Promote team spirit, cohesiveness, motivation, and commitment to service goals through engagement, empowerment, and transformational leadership
• Review and revise policies and procedures to ensure excellent delivery of service that aligns with Best Practice and patient outcomes
• Support staff in problem-solving and decision-making by ensuring administrative policies and procedures are followed
• Work collaboratively with the Program Director and other key interdisciplinary members to address operational issues that impact service delivery and efficient resource use
• Ensure excellence in patient care and service delivery through continuous quality improvement processes that include planning, quality monitoring, and risk avoidance
• Provide leadership in the development of new initiatives by creating an environment that encourages creativity, innovation, and critical thinking among the disciplinary team

Requirements

• Baccalaureate Degree in Nursing or other regulated Health Professional field
• Nurse Certificate of Competence from the College of Nurses of Ontario or another regulated Health Professional College
• Excellence in interpersonal, communication, coaching, mentoring, and team building skills
• Clear understanding of the application of the RHPA legislation
• Highly developed analytical, problem-solving, and critical thinking skills
• Proven ability to navigate challenging and sensitive discussions with patients, families, and care teams, exercising sound judgment, professionalism, diplomacy, and appropriate assertiveness
• Ability to introduce and effectively facilitate change
