Role Overview: Responsible for delivering professional, complex social services to patients and their families within an interdisciplinary setting.
Qualifications
- High School Diploma required; Master's of Social Work essential.
- Current licensure as a Licensed Clinical Social Worker (LCSW) or Licensed Master Social Worker (LMSW) in Texas.
- Basic Life Support (BLS) certification within 90 days of employment.
- SAFE Approach Level III certification within 6 months of hire.
Experience & Skills
- Preferred: At least 2 years of social work and counseling experience.
Key Responsibilities: Provide complex psychosocial assessments, develop care plans, coordinate services, and support patients and families through counseling and resource linkage, ensuring compliance with licensure and certification requirements.
