About the role
Role Overview: The Occupational Therapist in Home Health assesses and evaluates patients' functional status, activities of daily living, fine motor skills, and home environment needs to develop and implement personalized treatment plans.
Key Responsibilities
- Provide occupational therapy services within legal scope, following the plan of care and using evidence-based techniques.
- Evaluate patients' functional abilities, document CMS-required data, and collaborate with physicians on therapy plans.
- Monitor and report patient responses to treatment and any condition changes.
- Perform assessments to determine functional levels using diagnostic and prognostic tests.
- Adapt to diverse home environments and client backgrounds, ensuring effective care delivery.
Qualifications & Skills
- Current state licensure in Occupational Therapy.
- Valid driver’s license, vehicle insurance, or access to reliable transportation.
- Preferred: CPR certification, multitasking, independence, strong communication, organizational, and documentation skills.
