Community Healthcare Network (CHN)
This role as a Community Nutritionist involves working within a multidisciplinary team to provide nutrition intervention and counseling to a diverse patient population. The nutritionist will also plan, organize, oversee, and facilitate nutrition-related events, activities, and workshops.
Evaluate and monitor patients
Conduct nutrition education and counseling sessions
Organize and develop nutritional care plans and individualized therapeutic diets as needed
Complete necessary reports and forms and document patient services in the appropriate medical record
Facilitate related services for health center clients as appropriate with respect to their confidentiality and privacy
Organize and distribute nutrition education materials
Identify resources for nutrition education materials and services as needed
Conference with the Director of Nutrition as necessary to seek consultation and share information that is pertinent to patient care
Plan, conduct, and participate in nutrition education presentations, workshops, and health fairs for the agency
Attend conferences and participate in trainings/informal sessions as requested by the Director of Nutrition
Participate in clinic and central office team meetings as designated
Direct prenatal clients to local WIC offices and aid with their admission into the program
Perform other related duties, as assigned
