About This Role
Working with primary care and weight management providers to manage patients with nutrition-related conditions is the core of this role. The nutritionist will assess nutritional needs, educate individuals and groups, and develop educational materials to improve and maintain health within the community.
Responsibilities
- Work with primary care and weight management providers to manage patients with nutrition-related conditions
- Assess nutritional needs and food patterns of patients, families, and the community
- Educate and counsel children and adults on nutrition to improve and maintain health
- Plan and provide individual and group instruction based on patient needs assessments
- Develop nutrition educational materials and educate the community on nutrition topics in various formats in conjunction with the Health Education department
- Lead and coordinate with nutritionists to develop nutrition Quality Assurance program, Peer Review, and other quality improvement activities, update and review nutrition documentation template in EMR periodically
- Assist in supervisory and administrative duties of other nutritionists
- Assist other department heads in supervision and teaching of staff in delivering nutrition education and counseling
- Collaborate with care team to provide care management to patients with chronic diseases and high-risk patients
- Organize nutrition events with nutritionists and other staff
- Assist with collection and analysis of obesity and diabetes data and reports
- Assist research team on grant applications, data collection and analysis and clinical publications
- Provide training for other health center nutritionists, healthcare interns and Project Ahead students
- Assist with validation of UDS report and other relevant reports
- Other duties as assigned
Requirements
- Current New York State Certified Dietitian license and registration
- Certification by the Commission on Dietetic Registration
- Previous clinical diabetes management experience – preferably in community setting
Nice to have:
- Diabetes Educator Certification
