Nurse Practitioner - PRN

About This Role

This PRN Nurse Practitioner role involves conducting comprehensive health assessments for individuals in their homes or skilled nursing facilities. The position suits a compassionate and skilled nurse practitioner who can provide personalized care and identify chronic conditions that may otherwise go undiagnosed. You will work independently to improve health outcomes and lower costs for at-risk individuals.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up, and clear documentation according to Matrix guidelines and protocols
• Administer and order point of care and lab-based testing as needed
• Work with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate
• Collaborate with Primary Care Physician (PCP) on patient education and follow up as necessary
• Provide services at the member’s place of residence, which can be in their homes or Skilled Nursing Facilities

Requirements

• Master’s Degree or commensurate experience and satisfactory completion of NP licensure
• Current NP licensure in state of practice to include prescription authority or the ability to obtain prescriptive authority as needed
• AANP, AACN, or ANCC board certification
