This role involves delivering exceptional, team-based primary care directly to patients living with complex, chronic conditions and disabilities in Assisted Living and Memory Care communities. The Nurse Practitioner or Physician Assistant will manage a panel of patients, develop care plans, and collaborate with a multidisciplinary team to provide preventative and proactive care.
