Nurse Practitioner / Physician Assistant - Part Time

About This Role

This part-time role involves providing medical and primary care to residents in skilled nursing facilities. The Nurse Practitioner will assess and manage acute and chronic conditions, develop treatment plans, and collaborate with various healthcare teams. It suits an NP looking for flexibility, autonomy, and strong clinical support.

Responsibilities

• Assess, diagnose, and manage acute and chronic medical conditions
• Develop individualized treatment plans and provide ongoing patient care
• Complete timely clinical documentation and place appropriate orders
• Collaborate with physicians, nursing staff, and interdisciplinary care teams
• Maintain compliance with facility, state, and federal regulations

Requirements

• Active Nurse Practitioner (NP) license in Connecticut
• 2+ years of clinical experience preferred
• Experience in skilled nursing facilities, primary care, hospitalist medicine, or acute care preferred
• Strong clinical assessment and communication skills
• Ability to work independently and collaboratively with a clinical team
• Proficiency with EHRs and clinical technology

Important Details

• Schedule: Flexible, no nights, weekends, or holidays; workdays are scheduled Monday–Friday during normal business hours
