Job Description We are seeking a PRN/FLEX Nurse Practitioner with expertise in palliative care to provide comprehensive, individualized patient care in a home-based setting.

Key Responsibilities Include

Facilitating palliative care through diagnosis, treatment, consultation, and follow-up under medical supervision.

Developing personalized care plans demonstrating clinical excellence and ethical judgment.

Collaborating with a multidisciplinary team and maintaining compliance with healthcare regulations.

Qualifications And Skills Required MN Nurse Practitioner licensure, DEA license, and current RN license.

Minimum two years of NP clinical experience, preferably in palliative or hospice care.

Strong leadership, communication, decision-making, and self-motivation skills.

Proficiency with EMR software and willingness to travel 10-20% for training and education.

Education And Certifications

MSN or equivalent from an accredited institution.

Active Nurse Practitioner certification/license without restrictions.

Part B Provider certification, Federal DEA license, and collaborative physician agreement.

Preferred ACHPN Certification. Technical And Logistical Requirements

Competence in computer and smartphone use.
