Location: 3860 S Straits Hwy, Indian River, MI & 14719 W Upright St, Charlevoix, MI * Will split time between both locations *
Schedule: 40 Hours Weekly - Monday - Friday - Day Shift - 8AM - 4PM

A Day in the Life
- Act as a liaison between patients, professional staff and physicians by providing care management and coordination for adult and pediatric patients with complex conditions, social needs and educational needs.
- Serves in an expanded health care role to collaborate with primary care providers, specialists, and patient/families to ensure the delivery of quality cand cost-effective health care services.
- Assesses plans, implements, coordinates, monitors and evaluates all options and services with the goal of optimizing the patient's health status and decreasing hospital and emergency room utilization.
- Support quality improvement initiatives and adherence to safety standards.
What’s Required
- Licensed RN in the State of Michigan
- Experience in complex chronic disease management preferred
- Bachelor of Science degree in nursing preferred
- Five years' (5) experience as an RN preferred
