About the role
Role: Nurse Practitioner (NP) specializing in palliative care, working within a multidisciplinary team to provide consultation services in various healthcare settings, including nursing homes, hospitals, and community environments.
Responsibilities
- Implement and deliver specialist palliative care consultation services.
- Perform initial and follow-up patient visits, including symptom management, goal setting, and hospice eligibility assessments.
- Maintain effective communication with referral sources, patients, and families.
- Ensure accurate, timely, and compliant documentation of clinical encounters.
- Develop and implement care plans aimed at reducing illness burden.
- Contribute to quality improvement initiatives, staff education, and hospice operations.
- Participate in provider on-call rotations and perform face-to-face encounters as needed.
- Coordinate resources within an interdisciplinary team and travel within designated areas.
Qualifications & Skills
- Master's degree in Nursing and graduation from an approved Nurse Practitioner program.
- NY State RN licensure and certification as an Adult, Family, Gerontology, or Palliative Care Nurse Practitioner.
- Preferred minimum of 1-year clinical experience, especially in hospice, palliative, or geriatric nursing.
- Strong communication, teaching, listening, organizational, and critical thinking skills.
- Ability to work independently and collaboratively, demonstrating high accountability.
- Proficiency in Microsoft Word and Excel, with flexibility to work varied hours and travel within assigned areas.
- CPR-BLS certification and a valid NYS Driver's license are preferred; hospice & palliative care certification encouraged.
