About the role

Summary GENERAL SUMMARY The Mental Health Coordinator is responsible for assisting providers in an outpatient mental health setting in triaging and managing incoming referrals, providing case management services and have some clinical responsibilities. Reports to the Outpatient Mental Health Manager and designated Clinical Supervisor.

Outpatient ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Triages and manages referrals for Mental Health Service Line.
  • Inputs referrals in to our medical record and tracking system.
  • May provide non-contact clinical mental health interventions under direct supervision.
  • Facilitates comprehensive care management as related to bio-psychosocial components of illness, trauma, and disability.
  • Assists providers with case management needs as requested.
  • Supports in crisis intervention response.
  • Completes screening tools as directed for front-end assessments.
  • Maintains required records, statistics, and reports.
  • Participates in weekly didactic, staff, and supervision meetings.
  • Performs other duties as assigned.

Hospital Support

  • Facilitates bed searches and support to mental health assessment team.
  • Communicates with staff, patients and families regarding progress on status of placement, discharge, etc.
  • Facilitates court hearings for patients who are being held under a TDO.
  • Works within various systems to enter bed search updates, communication notes, and faxing referrals to outside hospitals, agencies.
  • Provides expertise and engagement with families for mental health referrals and resources.
  • Works in our scheduling system to get families scheduled with alternate level of care.
  • Participates in bi-monthly meetings with our clinical team.
  • Strong organizational skills are required.
  • Performs other duties as assigned.

LICENSES AND/OR CERTIFICATIONS None required.

Minimum Education And Experience Requirements

  • Bachelor’s degree from an accredited university is required. Degrees in psychology, social work, human services, or related therapeutic discipline preferred.
  • Qualified Mental Health Professional or at least one year of work experience in a mental health setting.
  • Enrollment in a Master’s Level Program from an accredited university with a degree in counseling, social, or psychology is preferred.
  • Exceptional interpersonal, oral, and written communication skills necessary.
  • Ability to work as a multidisciplinary team member.

WORKING CONDITIONS Normal office environment with little exposure to excessive noise, dust, temperature and the like.
