Medical Coder

About This Role

This medical coding role involves reviewing outpatient medical records and assigning accurate codes for billing and compliance. It suits a candidate with strong outpatient coding experience who can ensure alignment between documentation and assigned codes.

Responsibilities

• Review outpatient medical records and assign accurate ICD-10-CM, CPT, and HCPCS codes
• Ensure coding is compliant with CMS, AMA, NCCI edits, and payer-specific guidelines
• Code charts for outpatient services such as OPD visits, diagnostic services, minor procedures, therapy, labs, and radiology
• Validate medical necessity and ensure alignment between documentation and assigned codes
• Collaborate with providers to clarify documentation and improve accuracy
• Work with billing and AR teams to resolve coding-related denials and support appeals
• Conduct periodic self-audits to maintain high accuracy levels
• Stay updated with coding rules, annual guideline updates, and regulatory changes

Requirements

• Minimum 3 years of experience in outpatient coding
• Strong knowledge of outpatient coding rules and guidelines
• Expertise in ICD-10-CM, CPT, HCPCS Level II, and E/M outpatient guidelines
• Hands-on experience with EHR/EMR systems and coding platforms
• Ability to read and interpret provider documentation accurately
• Good understanding of outpatient billing workflow and payer policies
• Strong analytical and problem-solving skills
• Excellent communication and attention to detail
• 3+ years of work experience with CPT Coding
• 3+ years of work experience with ICD-10-CM
