Job Summary:
A well-established and highly regarded post-acute care organization is seeking an experienced MDS Coordinator (RN) to join its clinical leadership team. This is a confidential search for a key role responsible for ensuring the accuracy, compliance, and integrity of resident assessments and care planning.

Key Responsibilities:
- Complete accurate MDS assessments and comprehensive care plans
- Initiate and oversee care planning processes and related activities
- Develop and distribute monthly care plan schedules in a timely manner
- Maintain and update all clinical assessments in accordance with regulations
- Conduct ongoing audits of clinical documentation for accuracy and compliance
- Partner with nursing leadership to address and resolve documentation issues
- Ensure strict confidentiality of all resident and organizational information
Requirements:
- Prior MDS 3.0 experience required
- Active New Jersey RN License
- Previous Long-Term Care experience required
- Strong organizational and time management skills
- Proficiency with clinical and documentation systems
- Excellent communication and collaboration abilities
- Team-oriented with a professional demeanor
