Summary
- This position Provides patient/family education. Participates in family conferences. Responsible for all necessary documentation as required by accepted medical guidelines. This position (Outpatient\Inpatient) works under the supervision of Director. Evaluates, plans and implements treatment programs for the rehabilitation of patients, as directed by a provider, using approved treatment procedures.
- This position works under the supervision of the Director and is employed by Mosaic Health System.
Duties
- Evaluates patients referred by providers.
- Establishes goals that maximize the patient’s recovery.
- Plans a treatment program, using appropriate treatment procedures that will best accomplish the established goals.
- Implements the treatment program using approved procedures.
- Documents treatment procedures and the patient’s response to them, on a regular basis.
- Other duties as assigned
Qualifications
- All required education is a minimum requirement. Higher levels of education are acceptable. Bachelor's Degree- Graduate of accredited school of physical therapy required.
- Licensed Physical Therapist in State of Missouri required. Heart Saver certification or BLS required.
