Role Overview This position involves providing community-based nursing care, primarily in patients' homes, with responsibilities including assessing patient needs, developing care plans, and delivering personalized health services.
Responsibilities
- Performing comprehensive patient assessments and developing tailored care plans
- Providing direct nursing care and education to patients and their families
- Coordinating with multidisciplinary teams to ensure quality care
- Documenting patient information in compliance with HIPAA and organizational policies
- Supervising and collaborating with team members such as RNs, LPNs, PTs, OTs, RDs, SLPs, MSWs, and clinical schedulers
Qualifications & Skills
- BSN degree and at least two years of home care experience or equivalent education and experience
- Current RN license and registration in New York State
- Strong communication, organizational, and patient care skills
- Ability to work effectively in both office and patient home settings
- Knowledge of HIPAA regulations and patient confidentiality
