About the role
Role Overview: The Home and Community Based Services Licensed Practical Nurse provides direct nursing care to acutely and chronically ill patients in their homes.
Under supervision, you will deliver high-quality clinical services, document patient visits thoroughly, and participate in care coordination activities to ensure optimal patient outcomes.
Key Responsibilities
- Provide nursing care within the scope of practice, following the care plan and infection control procedures.
- Accurately document patient visits in electronic medical records and visit verification systems.
- Report patient needs and changes in condition promptly, participate in case conferences, and communicate effectively with patients, families, and healthcare team members.
Qualifications & Skills
- Current, unrestricted Licensed Practical Nurse licensure in the practicing state.
- CPR certification, valid driver’s license, and reliable transportation.
- Ability to work independently in diverse home environments regardless of patient background.
Preferred Attributes
- Home care experience, strong communication and organizational skills, and flexibility to meet patient needs.
- Ability to manage multiple tasks and work independently.
