About the role
Role Overview: The Home and Community Based Services Licensed Practical Nurse (LPN) provides direct nursing care to acutely or chronically ill patients in their homes, under supervision of a Registered Nurse or supervisor.
- Key Responsibilities: Deliver high-quality clinical services within legal scope, document patient visits accurately in electronic records, and participate actively in care coordination by communicating patient needs and changes in condition to the healthcare team.
- Qualifications: Hold an active, unrestricted LPN license in the state, possess current CPR certification, have a valid driver’s license with vehicle insurance, and demonstrate ability to work in diverse home environments. For Louisiana, at least 1 year of LPN experience is required.
- Preferred Skills: Flexibility to work varied hours, multitask effectively, work independently, and possess strong communication, organizational, and writing skills.
