Role Overview: The Licensed Practical Nurse (LPN) provides advanced clinical home care services, supporting patient recovery at home.
Key Responsibilities: Under RN supervision, perform patient assessments, medication management, and treatments such as urinary catheterization and sterile dressing changes. Document visits electronically within 24-48 hours, observe and report clinical data, and identify signs of health decline.
Qualifications & Skills: Must hold a NY State LPN license, with at least one year of Med/Surg experience in a hospital or long-term care setting. Requires a valid NY Driver's License, a personal vehicle, and good computer skills.
Additional Details: Revisit rates are $48 on weekdays and $55 on weekends/holidays. The role involves mapping out and overseeing patient recovery in the home, providing emotional support, and implementing care plans to ensure optimal health outcomes.
