Role:
Licensed Practical Nurse (LPN) providing advanced clinical home care services under RN supervision.
Responsibilities:
Perform patient assessments, medication management, and patient/family education; conduct home revisits based on clinical need; implement and evaluate care plans; document visits electronically within 24-48 hours; observe, measure, and report health status; recognize signs of health decline; administer medications and perform treatments such as urinary catheterization, dressing changes, and tracheal suctioning.
Qualifications and Skills:
NY State LPN license; minimum one year Med/Surg experience; valid NY driver’s license and own vehicle; strong computer skills; compassionate with clinical insight and decision-making abilities.
