About This Role

This full-time Licensed Practical Nurse position in an Assisted Living and Memory Care community involves administering medications, supporting wellness initiatives, and ensuring regulatory compliance and quality monitoring. Responsibilities

• Follow up on physician orders, coordinate services with ancillary providers, and report changes of condition or pertinent clinical care information to physicians and families
• Provide medication administration to an assigned group of residents according to physician orders and report any adverse reactions or clinical observations, documenting as necessary
• Ensure resident care and service plans are followed by overseeing tasks of Medication Technicians and care providers
• Make appropriate situational decisions and manage conflict in a positive and constructive manner
• Update and review resident medical records, clinical documentation, and incident reports

Requirements

• Active LPN license in the State in which this community is located
• CPR Certified or ability and willingness to obtain within 30 days of hire
• One year of medication administration experience preferred
• Ability to communicate clearly in verbal and written English
• Intermediate computer and mobile device skills
• Professional, pleasant, and team-oriented attitude
• Consistent and reliable attendance

Important Details

• Schedule: Full-time, 6:30am-7:00pm, includes every other weekend and 2 days during the week
• Weekend availability required
