Remote LVN Care Coordinator
Pay: $36.00–$40.00/hour
Location: 100% Remote (Candidate must live in California and have an active California LVN license)
Schedule : Monday–Friday, 40 hours/week
Hours : 8:00 AM–5:00 PM or 8:30 AM–5:30 PM PST
Assignment : Approximately 4-month W-2 contract
Target Start Dates: September 28 or October 5, 2026
Equipment : Client-provided equipment
Medix is hiring Licensed Vocational Nurses (LVNs) to support a California health plan with a regulatory compliance and care coordination project.
This is a fully remote, non-bedside nursing opportunity focused on clinical record review, care coordination, member documentation, and preparation of individualized care plans and case summaries.
The LVN will work under the supervision of a Registered Nurse and follow established Special Needs Plan (SNP) workflows to ensure assigned member cases are completed accurately and on time. Quality and productivity will be monitored daily.
Responsibilities
* Conduct focused data collection and basic assessments for assigned member cases
* Review clinical records, medication lists, encounter history, and current care plan status
* Draft components of Individualized Care Plans (ICPs) based on member needs and outreach under RN guidance
* Prepare SBAR summaries for RN review and Interdisciplinary Care Team (ICT) meetings
* Identify and escalate significant changes, clinical concerns, or red-flag findings to the supervising RN
* Support member outreach and care coordination follow-up within LVN scope of practice
* Maintain accurate and timely clinical documentation
* Follow established SNP, regulatory compliance, and care coordination workflows
* Meet established daily productivity and quality expectations
Required Qualifications
* Current California LVN license in good standing
* At least 1 year of professional LVN experience
* Strong clinical documentation skills
* Strong written and verbal communication skills
* Excellent attention to detail
* Ability to accurately review and organize clinical information
* Ability to follow established workflows and work independently in a remote environment
Preferred Qualifications
* 1+ year of experience in care coordination, case management, ambulatory care, managed care, or population health
* Health plan or Medicare Advantage experience
* SNP or D-SNP experience
* Experience reviewing medical records or charts
* Experience assisting with care plan development or implementation
* Familiarity with ICPs, ICTs, SBAR documentation, or member assessments
Additional Assignment Details
- This is an approximately four-month, project-based W-2 assignment through Medix supporting a regulatory compliance initiative. Permanent conversion is not anticipated at this time.
- Target start dates are September 28 and October 5, 2026. An earlier start may be possible depending on candidate availability, completion of required compliance onboarding, and equipment delivery.
- Employees are paid weekly through Medix. Benefits eligibility begins after 30 days, subject to applicable plan terms.
- The client will provide required work equipment, including a laptop and charger and potentially a headset (if needed).
