Qualifications & Experience Requirements for Licensed Social Worker:
- Previous Experience as a Social Worker preferred
- Social Worker License
Qualifications for Licensed Social Worker:
- Psychosocial assessments/BIMS/PHQ9/Trauma Induced Care
- Attend Care plans/meetings
- MOLST review
- Must have Knowledge of HCPOA/Advanced Directives, PASSAR/ID screens
- Work with the Interdisciplinary team with regards to discharge planning, MDS completion, formulate individualized care plans
- Provide resources PRN
- Advocate for our Seniors Aging in Place
- Attend meetings as appropriate
