Licensed Practical Nurse (LPN) Responsibilities:
- Provide direct patient care including administering medications, performing wound care, and assisting with ADLs
- Accurately document patient information, progress, and concerns in a timely and thorough manner
- Develop and implement individualized care plans to meet the unique needs of each resident
- Collaborate with interdisciplinary teams to ensure seamless communication and coordination of care
- Maintain a clean and safe environment, adhering to infection control and safety protocols
- Provide emotional support and comfort to residents and families during challenging times

Licensed Practical Nurse (LPN) Requirements:
- Current LPN licensure in the state of Pennsylvania
- Prior experience preferred (new graduates welcome to apply)
