Role Overview: The Home and Community Based Services Licensed Practical Nurse provides direct nursing care in patients' homes, following physician prescriptions.
Key Responsibilities

Deliver high-quality clinical services within the scope of practice, adhering to state laws and infection control protocols.

Document patient visits thoroughly in electronic medical records and visit verification systems.

Participate in care coordination, reporting patient needs and changes, and communicating effectively with patients, families, clinicians, and team members.

Work independently in diverse home environments, regardless of patient demographics.
Qualifications & Skills

Current, unrestricted LPN licensure in the state of practice.

Valid driver's license, vehicle insurance, or access to transportation.

Preferred: CPR certification, ability to work flexible hours, strong communication and organizational skills, and ability to manage multiple tasks.
