This role is for a Licensed Practical Nurse (LPN) providing patient-centered care in a primary care setting.
Responsibilities include
- administering vaccines, medications, and treatments
- managing patient communications via phone and telehealth
- supporting preventative and chronic care management
- collaborating with healthcare team members
- delivering care according to organizational policies and care plans
- participating in quality improvement initiatives
Qualifications include
- graduation from an accredited practical nursing program or passing the LPN Boards
- one year of recent LPN experience preferred
- excellent communication and teamwork skills
- computer proficiency, with EMR experience a plus
- current LPN license and BLS certification required
