About the role
Role: Licensed Practical Nurse (LPN) in a skilled nursing and rehab community.
Responsibilities
- Provide direct bedside care to residents, including administering medications and treatments.
- Monitor vital signs and resident conditions, documenting accurately in electronic records.
- Collaborate with RNs, physicians, and therapists to support personalized care plans.
- Educate residents and families about care and health management.
- Support activities of daily living (ADLs) and respond promptly to changes in resident conditions.
Qualifications & Skills
- Current valid LPN license.
- Compassionate, patient-centered approach.
- Strong communication and teamwork skills.
- Ability to document accurately and work collaboratively within a multidisciplinary team.
