As a Lead Social Worker, you will be responsible for the following duties:

Assess the psychological, emotional, social, economic, and physical impact of a patient's medical condition on their lives.

Provide counselling, crisis intervention, and emotional support to patients and their families.

Help patients and families access and utilize community resources, such as financial assistance, housing, transportation, and healthcare services.

Provide leadership and professional supervision to social workers and casework staff.

Manage complex cases through assessment, intervention, case planning and review.

Provide counselling and crisis intervention to clients and families.
-
Guide staff in applying evidence-based and strengths-based social work practices.
-
Conduct case reviews and ensure appropriate risk assessment and safeguarding.

Develop and maintain partnerships with community agencies and other stakeholders.
-
Lead groupwork, community initiatives and outreach programmes where required.
-
Collaborate with other healthcare professionals to develop treatment plans that address the patient's holistic needs.

Maintain accurate and confidential records of their interactions with patients and families.
Requirements:
- Minimum Degree in Social Work/Post Graduate Diploma in Social Work or equivalent.
- At least 3 years of relevant leadership experience.
