Job Title: Intake Nurse – RN (MLTC)

Location: Brooklyn, NY – Field-Based

Employment Type: Full-Time / Permanent

Salary: $100,000–$130,000 per year

Position Overview

We are seeking a Registered Nurse (RN) Intake Nurse to support the enrollment of new members into a Managed Long-Term Care (MLTC) program. This field-based role focuses on conducting initial assessments, evaluating eligibility and safety, identifying care needs, and helping members and their families understand available services.

The ideal candidate is an experienced RN with strong assessment and communication skills who enjoys working in the community and supporting older adults with complex healthcare needs.

Responsibilities

  • Conduct community outreach and intake activities as needed
  • Perform initial assessments using the UAS (Uniform Assessment System) tool
  • Evaluate potential members for program eligibility, safety, and care needs
  • Develop an initial plan of care based on assessment findings
  • Explain program services and requirements to potential members and their families
  • Coordinate with clinical, intake, and outreach teams to support a seamless transition into care
  • Communicate with members, families, referral sources, and physicians
  • Complete enrollment and clinical documentation accurately and timely
  • Serve as a community resource for intake and care-related questions
  • Consider each member’s age, developmental, mental, social, and cultural needs when conducting assessments
  • Maintain knowledge of Medicare and Medicaid requirements
  • Perform other job-related duties as assigned

Qualifications

  • Active New York State RN license required
  • Associate’s degree in Nursing required; Bachelor’s degree preferred
  • Minimum 1 year of acute care nursing experience required
  • Home care experience preferred
  • Managed Long-Term Care (MLTC) experience preferred
  • UAS assessment experience preferred
  • Strong clinical assessment, critical-thinking, and problem-solving skills
  • Excellent verbal and written communication skills
  • Ability to work independently while collaborating effectively with a multidisciplinary team
  • Knowledge of Medicare and Medicaid regulations
  • Bilingual or multilingual skills are a plus, particularly Spanish, Russian, Mandarin, or Cantonese
  • Must be comfortable working in a mobile, community-based environment

Important Details

  • Travel: 50%+ travel within the assigned territory
  • Territory: Candidates must reside in or near their assigned territory
  • Driver Requirement: Valid driver’s license, personal vehicle, and verifiable auto insurance required
  • Schedule: Flexible hours; occasional evenings and/or weekends may be required based on member needs
  • Salary: $100,000–$130,000 annually

  • Work Environment: Regular travel to members’ homes, offices, and community settings; exposure to typical outdoor weather conditions
