About the role
Role Overview: As an LPN Intake Coordinator, you will handle incoming referral and order calls, gather and verify patient demographic and clinical information, and ensure accurate communication with relevant teams.
Key Responsibilities
- Obtain and compile referral data from clients, physicians, and referral sources
- Verify physician licenses and research coverage or pay sources for referrals
- Educate patients, families, physicians, and hospitals about home health services
- Act as a liaison between clinical and marketing teams
Qualifications
- Current, unrestricted LPN license in the practice state
- At least 1 year of insurance or medical experience
- Valid driver’s license, vehicle insurance, and access to transportation
Preferred Skills: CPR certification or ability to obtain within 90 days
