About This Role
This Information & Referral Specialist role provides vital information to consumers regarding service and eligibility inquiries, supporting older adults and individuals with disabilities. The specialist assesses health and functional status, develops care plans, and coordinates services to promote healthy, safe living within their communities.
Responsibilities
- Assess the health and functional status of members
- Develop community-based care plans and related service packages
- Participate in the development of a member's Individual Care Plan (ICP)
- Arrange, coordinate, and monitor the provision of appropriate members
- Participate in the determination of a member's need for institutional and community long-term care services
Requirements
- Passion for helping others and desire to make a difference
- Minimum of two years' professional experience in the care of people 65 and older
- Working knowledge of social, emotional, and physical process of aging
- Ability to obtain information of a personal nature through interviews
- Ability to gain and hold the confidence of clients and their families
- Draw appropriate conclusions and make recommendations
- Knowledge and skill necessary to work successfully within an interdisciplinary team
- Excellent written and verbal skills
- Excellent computer skills
- Proficient with Microsoft Office Suite or related software
Nice to have:
- Bachelor's degree in social work or related field
- Current Social Work license (LSW) or higher
