About This Role
This nursing post focuses on providing after-hours hospice care and clinical support to patients and families during weekend coverage. The role suits a dedicated Registered Nurse who can work independently and provide compassionate end-of-life care.
Responsibilities
- Triage after-hours calls and assess patient needs
- Provide clinically appropriate home visits, including symptom management, crisis, death, and other urgent, unplanned nursing visits
- Complete hospice referrals, admissions, and starts of care during after-hours coverage when immediate hospice services are needed
- Deliver skilled nursing care according to the patient's plan of care and physician orders
- Monitor patient conditions and communicate significant changes to the interdisciplinary team
- Coordinate care across settings, including inpatient, respite, and routine home care services
- Maintain accurate and timely clinical documentation
- Demonstrate professionalism, empathy, teamwork, and exceptional customer service in all patient and family interactions
- Cover areas in Clarendon, Florence, Cheraw, and Dillon
Requirements
- Current South Carolina RN License
- Current South Carolina Driver
essential: License and automobile insurance as required by law
- Current BLS Certification
- Active patient care experience within the past year
- Minimum of 1-2 years of RN experience

- Schedule: 72 hours biweekly, Friday 11:00 AM – 11:00 PM, Saturday 7:00 AM – 7:00 PM, Sunday 7:00 AM – 7:00 PM
