Role: Hospice RN Case Manager
Responsibilities
- Coordinate and deliver end-of-life care in patients’ homes and facilities.
- Manage a caseload of hospice patients, conducting comprehensive assessments.
- Develop, update, and implement individualized care plans.
- Provide skilled nursing, pain and symptom management, and patient/family education.
- Collaborate with physicians, social workers, chaplains, and aides to ensure coordinated care.
- Ensure timely visits, accurate documentation, and regulatory compliance.
- Support families throughout the hospice journey.
