Hospice Nurse RN - Home Hospice

About This Role

This nursing role involves providing comprehensive hospice care to individual patients, focusing on physical assessment, pain and symptom management, and coordinating services. The position suits a compassionate Registered Nurse who can establish therapeutic relationships with patients and families, and who is capable of managing care and documentation in a timely manner.

Responsibilities

• Assume patient care responsibilities for individual patients
• Complete comprehensive physical assessment
• Conduct pain assessment and management
• Manage symptom control as it relates to terminal illness
• Coordinate Circle of Life services to the patient/family
• Complete required documentation (i.e., assessments, care plans, clinical notes, agency tracking forms, etc.) in a timely manner in accordance with COL policies
• Establish a therapeutic nursing relationship with the Hospice patient/family/caregivers
• Provide weekend coverage and may assist during times of excessive nursing absenteeism
• Assume primary responsibility for assigned patients and perform all phases of the nursing process
• Communicate with attending physicians, staff members, and other agencies as necessary to coordinate optimal care and use of resources for patients/caregivers
• Educate patients/caregivers regarding hospice philosophy, disease progression, physical components of care, stages of dying, and other aspects of the POC as necessary
• Document all patient related information according to policies and procedures
• Seek input from IDT members regarding the patient POC to obtain additional knowledge and support
• Review and update the POC at least every 60 days per regulatory standards
• Regularly attend and participate in IDT meetings to ensure that the POC reflects current patient care needs
• Prepare in advance for IDT meetings
• Assess need for Certified Nursing Assistant services
• Conduct CNA supervisory visits per regulatory standards
• Assess the need for and revise the CNA care plan
• Inform the PCM of unusual or potentially problematic patient/caregiver issues as identified
• Maintain annual clinical competencies
• Identify patients who may potentially revoke or need to be discharged
• Model prompt and regular attendance
• Exhibit cost effective use of time, mileage and supplies
• Demonstrate familiarity with and adhere to policies and procedures, state, federal, and accrediting agency's requirements
• Provide notification of absence per policy
• Promote and maintain respect for rights, privacy, and confidentiality of others
