Role: Hospice Medical Social Worker
Responsibilities
- Conduct psychosocial assessments of patients and families facing serious illness.
- Develop personalized care plans and provide counseling, crisis intervention, and grief support.
- Connect families with community resources and guide advance care planning.
- Advocate for patient preferences and ensure patient-centered care.
- Collaborate with an interdisciplinary hospice team.
- Document care activities, attend team conferences, and make home and facility visits.
Qualifications & Skills
- Compassionate with excellent communication and interpersonal skills.
- Ability to provide emotional support and crisis intervention.
- Strong organizational and documentation skills.
- Experience in hospice or end-of-life care preferred.
