Hospice Admissions RN

Join a compassionate, mission-driven team with Hospice of Marion County, part of Empath Health. We’re looking for a dedicated and empathetic Admissions RN to make a meaningful impact by guiding patients and families through the admissions process—providing care and support in homes, healthcare facilities, and hospitals throughout Marion County.

Schedule: PRN, minimum of 64 hours a month availability. Hospice experience required.

What you'll Do

• The Admissions RN is a first responder who assesses eligibility for Empath Health Network of Care.
• Advocating for optimal access to the right care at the right time in any care setting.
• The Admissions RN completes the admission process based on care program specifics, guidelines, and patient-specific needs.
• Coordinate and assist with the Professional Liaison to deliver educational, in-service and outreach tools in the hospital.
• Works directly with community referral sources to educate regarding Empath Health programs and services.
• Complies with RN On Call scheduling needs as well as vendor credentialing process.

What You'll Need

• Hospice experience strongly preferred
• Education and/or Experience: Associate’s degree (A.A.) or equivalent from two-year college or technical school; or six months to one year related experience and/or training; or equivalent combination of education and experience.
• Licensure:
  • Valid Florida license as a Registered Nurse.
  • Valid Florida State Driver’s License.
• For field-based positions, employees must have reliable transportation which will enable them to perform tasks and responsibilities in a timely and appropriate fashion. Must provide proof of valid automobile insurance, a copy of which will be placed in the employee’s HR file.
