Role Overview: The Home Provider supports individuals with intellectual disabilities by opening their home and creating a welcoming, safe environment.
Key Responsibilities: Assist with daily living skills such as cooking, cleaning, budgeting, and decision-making; support medication management and medical appointments; facilitate community participation and social connections; provide transportation and help with personal care and behavioral support as needed; maintain documentation and communicate with the support team; promote independence and confidence.
Qualifications & Skills: Must be at least 21 years old, a resident of Illinois, own or rent a home, have active insurance, reliable transportation, internet access, and basic computer skills. Must be a registered healthcare worker (DSP) and able to complete screenings, background checks, and training.
