Role Overview: The Medical Social Worker provides social services under physician and interdisciplinary team guidance to address social and emotional factors impacting patient health.
Key Responsibilities
- Assist patients, families, and healthcare providers in understanding personal, emotional, and social aspects related to health status.
- Contribute to developing and implementing comprehensive, integrated care plans.
- Educate healthcare team members on community resources to support patient needs.
- Function as Bereavement Coordinator, supervising bereavement services and creating tailored care plans for grieving patients and families.
Qualifications & Skills
- Master's Degree in Social Work from an accredited program.
- State licensure as a Licensed Social Worker.
- Current CPR certification and a valid driver’s license with access to transportation.
- At least one year of social work experience in a healthcare setting.
Preferred Skills: Experience in bereavement coordination and planning.
