Role Overview: Serve as a primary clinical partner delivering individualized home nursing care to promote recovery, safety, and independence.
Key Responsibilities
- Conduct comprehensive in-home assessments to develop patient-centered care plans.
- Provide skilled interventions such as wound care, infusions, medication management, and post-operative care.
- Educate and empower patients and caregivers to manage symptoms and treatments confidently.
- Monitor patient progress, identify changes early, and communicate with the care team to adjust plans accordingly.
- Coordinate care continuity across providers, ensuring high-quality outcomes.
- Complete accurate documentation to support compliance and measurable results.
Qualifications & Skills
- Active RN license in practice state.
- At least one year of clinical nursing experience.
- Patient-focused, organized, and motivated to deliver quality care.
- Valid driver’s license, reliable transportation, and CPR certification or ability to obtain.
