About the role
We are seeking a dedicated Registered Nurse (Home Health) to provide personalized, one-on-one nursing care in patients' homes. Responsibilities include conducting comprehensive assessments, delivering skilled interventions such as wound care, infusions, medication management, and post-operative care, and educating patients and caregivers to promote independence and safety. The role involves monitoring patient progress, communicating with the care team, coordinating services, and ensuring accurate documentation to support quality outcomes and compliance.
Qualifications
- Active RN license in the practice state
- At least one year of clinical nursing experience
- Patient-focused, organized, and motivated to deliver high-quality care
- Valid driver’s license, reliable transportation, and auto insurance
- CPR certification or ability to obtain it
