About the role
Home Health Physical Therapist Provide patient-centered care to geriatric patients in a home setting, conducting in-home evaluations and developing individualized plans of care. Focus on therapeutic interventions such as functional strengthening, gait training, balance, and fall risk reduction to enhance mobility and independence.
Responsibilities Include
- Managing your caseload and scheduling visits based on your availability and patient needs.
- Collaborating with a multidisciplinary team to ensure coordinated care and optimal patient outcomes.
- Utilizing an innovative app for managing logistics, documentation, and communication, streamlining workflow and administrative tasks.
- Ensuring compliant charting with integrated EMR system and maintaining all necessary certifications and licenses.
Qualifications And Skills
- Valid California Physical Therapist license
- CPR or BLS certification
- Driver's license and auto insurance
- Physical and TB test (within the last year)
- Professional liability and business license (if applicable)
