About This Role

This home health physical therapist position involves delivering quality healthcare to patients of all ages within their homes as an independent contractor. The role offers flexibility and autonomy, allowing the therapist to manage their own schedule and patient load.

Responsibilities

• Deliver quality home healthcare to patients of all ages
• Observe, record, and report the patient's response to treatment and changes to the patient's condition
• Determine the plan of care
• Complete documentation using an intuitive Electronic Medical Record system
• Accept or decline patients using the Cliniconnects app

Requirements

• Ability and enthusiasm in working with all ages
• Current, active, and unrestricted Physical Therapist license issued by the state Board of Physical Therapy
• Completion of an accredited Physical Therapy program
• Reliable personal transportation
• Smartphone
