About the role
Position Overview Seeking an Occupational Therapist (OT) to assess, teach, and deliver therapy interventions in a home health setting. The role involves collaborating with an interdisciplinary team, providing updates to optimize patient well-being, and managing care plans.
Key Responsibilities
- Perform initial and ongoing assessments of mobility, coordination, strength, and function.
- Administer, evaluate, and interpret tests to set functional goals and outcomes.
- Develop and document individualized plans of care, including goals, progress, and discharge plans.
- Provide occupational therapy services following physician orders.
- Maintain accurate documentation to ensure compliance and support effective communication.
- Coordinate with healthcare team members, patients, and caregivers.
- Supervise other personnel such as OT Assistants or Home Health Aides as applicable.
- Complete assessments like OASIS where permitted by regulation.
Qualifications & Skills
- Degree in Occupational Therapy from an accredited program.
- Current, unrestricted state license as an OT.
- Valid CPR certification.
- Valid driver’s license and proof of automobile insurance.
Preferred Experience: Home health therapy experience and OASIS documentation familiarity.
